Hydroxocobalamin

Hydroxocobalamin is a cobalamine derivative in which the substituent is a hydroxyl group. Hydroxocobalamin is the natural form of the vitamin B12 and is sometime used as a source of vitamin B12.


Adult Dose
Dose: 250 to 1000 mcg
Single Dose: 620 (625)
Frequency: As recommended.
Route: IM
Instructions: Every alternate day for 1-2 weeks, Maintenance dose is given monthly.
Neonatal
Dose: 1 mg
Single Dose: 1 (1)
Frequency: As recommended.
Route: Intra Muscular
Instructions: Initially 5 doses at 2-3 days interval
Paedriatic
Dose: 1 mg

Characteristics
Hydroxocobalamin also known as Vitamin B-12. . It is of Semi Synthetic origin and belongs to cobalamine. It belongs to Nutrients and Vitamins pharmacological group on the basis of mechanism of action and also classified in Hematological Agents pharmacological group.The Molecular Weight of Hydroxocobalamin is 1346.00. Its pKa is not relevant.
Contraindications
Hydroxocobalamin is contraindicated in conditions like Anaphylactic reactions.
Effects
The severe or irreversible adverse effects of Hydroxocobalamin, which give rise to further complications include Hypersensitivity reactions.Hydroxocobalamin produces potentially life-threatening effects which include Cardiac arrhythmia, Allergic reactions, Anaphylactic reactions, Hypocalcemia. which are responsible for the discontinuation of Hydroxocobalamin therapy.The symptomatic adverse reactions produced by Hydroxocobalamin are more or less tolerable and if they become severe, they can be treated symptomatically, these include Headache, Diarrhea, Nausea and vomiting, Hypokalemia, Red urine, Pain at injection site, Pain at injection site.
Indications
'Hydroxocobalamin is primarily indicated in conditions like Anaemia, Congenital if deficiency, Congenital intrinsic factor deficiency, Ileal resection, Methylmalonylacidurias, Nutrition deficiency, Pernicious anaemia, Pernicious anaemia and other macrocytic anaemias, Prophylaxis of macrocytic anaemias, Thyrotoxicosis, Tobacco amblyopia, Tobacco amblyopia and leber''s optic atrophy, Transcobalamin II deficiency, Vitamin deficiency, and can also be given in adjunctive therapy as an alternative drug of choice in Cyanide poisoning, Homocystenemia, Infected burns.'
