Ciprofloxacin

Ciprofloxacin is a fluoroquinolone antimicrobial agent with a broad spectrum of activity against both gram-negative and gram-positive bacteria.They are relatively non-toxic and well tolerated broad spectrum agents. Their excellent bioavailibilty permits their use for treatment of variety of serious bacterial infections. Antibiotics require constant drug level in body for therapeutic effect. This is achieved by taking the medication at regular interval of time throughout the day and night as prescribed. Ciprofloxacin (HCl) is important to take the drug for the full time period as prescribed. If you discontinue the therapy it may result in ineffective treatment.

Ciprofloxacin (HCl) is the derivative of Ciprofloxacin. It is of Semi Synthetic origin and belongs to Quinoline Carboxylic Acid. It belongs to DNA Gyrase inhibitor pharmacological group on the basis of mechanism of action and also classified in Antibiotics pharmacological group.The Molecular Weight of Ciprofloxacin is 346.00. Its pKa is 6-8.8.
Contraindications
Ciprofloxacin is contraindicated in conditions like Epilepsy,Hypersensitivity.
Effects
The severe or irreversible adverse effects of Ciprofloxacin, which give rise to further complications include Pseudomembranous colitis, Hearing loss, Seizures.Ciprofloxacin produces potentially life-threatening effects which include Anaphylactoid reactions, Stevens Johnson syndrome, Acute Renal Failure, Fluminant hepatic failure, Epidermal necrolysis. which are responsible for the discontinuation of Ciprofloxacin therapy.The symptomatic adverse reactions produced by Ciprofloxacin are more or less tolerable and if they become severe, they can be treated symptomatically, these include Dizziness, Headache, Nausea, Vomiting, Diarrhea, Tachycardia, Nervousness, Termors, Rashes, Urticaria, Pruritus, Photosensitivity, Elevation of liver enzymes, Thrombocytopenia, Eosinophilia, Increased intracranial pressure, nervousness, joint symptoms, agranulocytosis.
Indications
Ciprofloxacin is primarily indicated in conditions like Acromegaly (short-term treatment before pituitary surgery), Chronic prostatitis, GI infections, Gonorrhoea, Iron-deficiency anaemia prophylaxis or for mild iron deficiency, Iron-deficiency anaemia with chronic renal failure, Most other infections, Muscle relaxation (long duration) for surgery and intubation, Muscle relaxation for surgery or intubation, Osteomyelitis, Pseudomonal lower+RTI in cystic fibrosis, Respiratory tract infections, Sexually transmitted disease, Sexually transmitted infections, Skin infections, Skin structure infections, Status epilepticus (unlicensed use), Surgical prophylaxis, Urinary tract infection.
Interactions
Ciprofloxacin is known to interact with other drugs, the details of drug interactions is as follows:DrugDetailsSeverityOnsetManagementCaffeineCoadministration inhibit the metabolism of caffeine thus increasing its plasma concentration and pharmacological effect.Ciprofloxacin increases caffeine AUC by 50% to over 100% and reduces its clearance upto 50%.ModerateCaffeine intake should be avoided during treatment that may increase the risk of adverse effects like tremor,insomnia,tachycardia and blood pressure.Calcium Carbonatebinds with drugs or increase pH that effects drug solubilityantacid should not be given within two hours of the dose of these drugsChloramphenicolClozapineCoadministration increases the plasma concentration of clozapine by inhibiting CYP450 1A2 responsible for metabolic clearance of clozapine.ModeratePharmacological response and plasma clozapine concentration should be monitored closely.Cyclosporin ACiprofloxacin increases the serum concentration of cyclosporin. The risk of cyclosporine induced neurotoxicity and nephrotoxicity may also be increased.ModerateCyclosporine levels and renal function must be closely monitored.Insulin Lispro ProtamineMagnesium Oxides and HydroxidesMagnesium hydroxides and aluminium hydroxide antacids, iron, calcium, zinc containing products impair absorption of ciprofloxacin.Mangnese Glycerophosphate Mangnese decreases effectiveness of Quinolone antibioticsModerateTake manganese supplements at least one hour after antibiotics.MethotrexateCiprofloxacin increases the plasma concentration of methotrexate by competitively inhibiting renal tubular secretion of methotrexate result in severe toxicity.ModerateConcomitant use of these drugs considered contraindicated. Closely monitor for altered pharmacologic effects. Adjust the dose if necessary.Multiple VitaminsConcurrent use may decrease the absorption of Ciprofloxacin.Separate your doses of Ciprofloxacin as far as possible from doses of multi vitamins.NaproxenPiperazine ring of ciprofloxacin inhibits the binding of GABA to brain receptors and naproxen synergistically add to this effect thus increase the risk of CNS stimulation and convulsion.ModerateClose monitoring of CNS stimulation is advised.PentostatinConcurrent use may reduce the plasma concentrations of oral quinolone antibiotics.MinorDuring concomitant use with chemotherapeutic agents, patient response to quinolone therapy should be monitored and the dosage of antibiotic adjusted accordingly if necessary.Phenytoin (Na)Ciprofloxacin may either increase or decrease the plasma concentration of phenytoin, thus increasing the risk of phenytoin toxicity or seizure.ModerateClosely monitor the plasma concentration of phenytoin whenever start or discontinue ciprofloxacin. Adjust the dose of phenytoin accordingly.PomalidomidePomalidomide exposure is increased co-administered with ciprofloxacinAvoid co-administration, if medically necessary Pomalidomide dose should be reduced by 50%.ProbenecidProbenecid decreases the urinary excretion of ciprofloxacin thus increases its plasma concentration.MinorRifampicinRifampin induces the metabolism of ciprofloxacin.Rutin (Vitamin P)Rutin may competitively inhibit action of Ciprofloxacin.Sevelamer HClsevelamer hydrochloride decreased the bioavailability of ciprofloxacin by approximately 50%.Should not be taken simultaneously with ciprofloxacinSucralfateTheophyllineCiprofloxacin enhances the serum level of theophylline by inhibiting its metabolism and by reducing its clearence upto 65% may result in increased risk of toxicity. Concomitant use of theohylline with interavenous ciprofloxacin may results cardiac arrest, seizure,status epilepticus and respiratory failure.MajorConcomitant use should be avoided. Closely monitor the pharmacologic response and serum level of theophylline whenever start, discontinue or change the dose of ciprfloxacin.Patient should notify the symptoms of theophylline toxicity.Warfarin (Na)Ciprofloxacin potentiate the hypoprothrombinemic effect of warfarin by inhibiting its metabolism and depleting certain clotting factors due to suppression of vitamin-k producing intestinal flora.MajorINR should be checked frequently and dose of warfarin should be adjusted whenever start or discontinue quinolone therapy. Patient should report any sign of bleeding to doctor. These interactions are sometimes beneficial and sometimes may pose threats to life. Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.

Risks
Drug should not be given to Paediatrics, Pregnant Mothers, Cardiac / Hypertensive Patients, patients suffering from Kidney dysfunction, patients suffering from Liver Malfunction, and Neonates.If prescribing authority justifies the benefits of the drug against the possible damages he/she should reevaluate them and consult the reference material and previous studies.

Warnings
Ciprofloxacin should be used with caution in patients with known or suspected central nervous system (CNS) disorders, renal impairment or hepatic disease, with a history of GI disease especially colitis or who have dehydration. Do not exceed the recommended dose. Take appropriate measures if secondary infections occur.
